Assist with On-Call

Essential Functions: 

1.    Follow established standards of nursing practice and implement facility policies and procedures.
2.    Interpret existing policies and procedures to Resident Care Technicians, residents, families and physicians.
3.    Supervise and evaluate all direct resident care and initiate corrective action as necessary.
4.    Receive report from the nurse you are relieving and record sufficient information to implement follow-up action as necessary.
5.    Provide report to nurse coming on duty., including sufficient information for follow-up action if necessary.
6.    Conduct resident rounds daily, reporting problems to the charge nurse or Director of Nursing, if necessary, and initiate corrective action.
7.    Identify resident problems, emergency situations and respond appropriately within the Quality of Life Policies of Healthcare of Iowa.
8.    Identify safety hazards and take appropriate action.

9.    Maintain MDS schedule to meet both Medicare and OBRA requirements. Keep all disciplines informed of schedule and schedule changes.
10.    Timely and accurate completion of MDS assessments. Assessments consist of residents, family, and staff interviews, physical assessments and medical record review.
11.    Timely submission of MDS assessments to the QIES system.
12.    Completion of Baseline Care Plans upon admission. Development and timely updates of resident comprehensive care plan.
13.    Conduct bedside Admission Nursing Assessment, Braden Assessment, Fall Risk Assessment, Side Rail Assessment, Active TB Assessment, Admission Section GG Assessment on day of admission.
14.    Review admission chart for completion.
15.    Audit skilled charts for completion of required documentation.
16.   Gather information and facilitate Medicare meeting. Communicate discharge plans to interdisciplinary team.
17.    Follow, supervise, and evaluate the implementation of the Resident’s Rights Program.

Requirements

Education:
Graduate of an accredited Registered Nurse or LPN program.
Licensure/Certification in the state of lowa required.
